None. It's always recursive.

Yes it does, it gives you user/group information, filenames which are
more than 8.3 letters uppercase, and saves the permission flags as well.
On top of that, you can have symlinks and special files (devices,
sockets). Hardlinks don't work properly.

mkisofs -o somewhere Main_dir

Will create an image with Main_dir/. (i.e. the contents of Main_dir) as
top-level directory. I have 1.11.a06. They have all worked like that.
