You might want to check that the pointer with the "ZAxisMapping"
option is actually being used as the default.  It is not uncommon for
more than one pointer to be specified in XF86Config-4.

I'm afraid I don't have access to a Linux machine at work, so I can't
tell you exactly the string to look for, but the config file is
somewhat self-explanatory.

Restarting X (Ctrl-Alt-Backspace after closing all apps) can also help
solve mouse problems sometimes, although it seems unlikely in your
case.
