Hello,
I have written some software that is not completely applicable to your
problem (its written in Java) but it does do a fairly comprehensive job
of contact management.
It uses a MySQL database with an LDAP-based authentication and
permissions system (it even automatically logs you in if you are
running within a managed Novell network environment).

It manages people, organisations and groups. You can give individuals
and infinite number of addresses, phone numbers, email addresses,
interests and memos.
There is quite a few other features and it looks fairly tidy.
Information can be exported to PDF (info sheets, envelopes and address
stickers), csv and vcards.
Its written in Java and runs within Tomcat or similar JSP engine (it is
tried and in production of Novell, Linux and Windows servers).
Its not written in PHP but it could be useful if you are a little
flexible.
