I have a sneaking suspicion it could be a USB-controller problem. I
had machine which was loading the uhci usb driver when it should have
been loading ohci. When I finally configured it to load ohci, there
was a subtle driver bug which stopped it recognising a particular USB
key. The key worked fine on other machines, which all had
uhci-compatible cards.
