A bug? in my perfect code? 

SURELY NOT! ;-)

OK, that sounds like openpsfc scanned the first page, but didn't end up
moving the tiff file to the final output, or was unable to properly scan
the document.

You should be able to scan in at least a single page without writing any
config for your model scanner yet.

Can you please check if the following programs are installed:
* /usr/bin/scanimage	(from SANE)
* /usr/bin/tiffcp	(from libtiff)
* /usr/bin/tiff2pdf	(from libtiff)

And let me know:
* Version of PHP installed - assuming v5?
