It may be an auto negotiation issue with the switch, check what the card reports when its not working with mii-tool. 

If this is the problem you can try manually setting these settings this using

mii-tool -F 100baseTx-FD

you should beable to do this on the switch aswell on a cisco do (example is using port 6 on the switch):

en
show running-config | begin 0/6
conf t
int fa0/6
speed 100
duplex full
exit
copy run start

other switches should have similar commands.

if this is the problem then you will probably want to add the mii-tool command to /etc/rc.conf so its set at boot.
