I generally use old slim line desktops with lan built into 
motherboard.  Smallest I've had was 300mm wide by 80mm high but I've seen 
smaller  - stick it under your main computers monitor instead of those 
telephone books :-)
No HDD, no CPU fan, not a huge power consumption so power supply fan is 
just idling (assuming a temp controlled fan, else just wire it to the 5v 
line and run at half speed) = little noise.
(and I bet you're not paying for the power :-)
