We're using Asterisk at Home, which is a specialised distro based on CentOS
that integrates Asterisk and SugarCRM.

I'm pretty sure you can get asterisk to forward outward VoIP through
Skype, which is pretty cheap. We haven't done it, but they
(asterisk at home) have written a call feature in Sugar that hooks up to
Skype.
