Good idea, you are quite right, it should be done :-) In the words of the cabal
first amongst equals (aka committee/LUG president, aka Jamie B.): Go for it!!

Here are some suggestions/questions, mostly good/OK/bad or yes/maybe/no with
somewhere for a comment if desired:

I suggest any WLUG members that attended also fill it out, & the next time we
have such things ready to be filled out there to capture info from those who
didn't register.


Add/delete/modify the questions as y'all see fit to get summat we are all
comfortable with (assuming you agree it is a good idea...)
